This activity is entirely voluntary and the Rhoer may withdraw from the activity at any time. In
deciding whether you will participate in the activity, you should be aware that the Sorority cannot
undertake financial responsibility for you in the event that you are injured while participating in
the activity.
As in the case with many other activities that you may engage, you should determine whether
you or your family have health insurance in effect to adequately cover you should you be injured
while participating in this activity.
I have read and understood the foregoing explanation of the risk inherent in the activity. As the
parent/guardian of this minor, I have given my permission for the Rhoer to participate in the
activity. I hereby release Sigma Gamma Rho Sorority, Inc. from any and all liability and claims,
which I may have against it for loss or damage to property resulting from my child’s participation
in the activity.
