When your Free Application for Federal Student Aid (FAFSA) or Dream Act Application was processed, your Selective Service registration or
exemption status was not confirmed by the Selective Service system. As a result, you must provide documentation as proof of your registration or
exemption status.
No financial aid awards or disbursements will be made until appropriate documentation is received.
Check the appropriate box and sign below, attach any supporting documents required, and submit this form to the Financial Aid & Scholarships office.
I am already registered with the Selective Service. Complete the following:
1. Visit the Selective Service website at www.sss.gov and choose the “Check Registration” option
2. View and then print proof of your registration. Submit this form and proof of registration to the Financial Aid & Scholarships office
I am not registered with the Selective Service but meet one of the following exceptions listed below
I hereby certify that I am exempt from registration with the Selective Service because (check one option below):
I am currently in the armed services on active duty
(Does not apply to non-activated members of the Reserves or National Guard)
I have not yet reached my 18th birthday
I am not registered and do not meet one of the above exceptions. I hereby certify that (check one option below):
I am between the ages of 18 and 25 and required to register with the Selective Service at www.sss.gov
Please attach documentation to verify that you have now registered
I am 26+ years old and I was 26 or older when I first entered the U.S.
Please attach documentation to verify the date you first entered the United States
I am 26+ years old, I entered the U.S. on a valid non-immigrant visa prior to turning age 26, and I remained in that status
until after my 26th birthday. Please attach documentation to verify your visa status at the time you turned 26.
I am 26+ years old and do not fall into one of the above categories. Complete the following:
1. Go to www.sss.gov and follow the directions to request a "Status Information Letter"
2. When you receive the Status Information Letter from the Selective Service, submit a copy to the Financial Aid & Scholarships office
along with this form. If the Status Information Letter DOES NOT verify that you were exempt from registering, you must also submit a
typed and signed statement explaining why you failed to register, please use the "Financial Aid Statement" form.
