• Use this form to notify Schwab of the death of the last surviving Designated Beneficiary Plan account holder and to
instruct Schwab (and Schwab Bank, if applicable) on how to distribute the decedent’s total assets. This form should be
completed by the decedent’s individual beneficiary(ies) or the guardian or custodian of a beneficiary who is a minor.
Each beneficiary must complete a separate form.
• Required documentation. Please attach the following to this form:
– A copy of the account holder’s death certificate
– A notarized Affidavit of Domicile
– A tax waiver, if required by state law
• If you are a nonresident alien without a Social Security number, you will need to complete IRS Form W-8BEN and send
it with a certified copy of your passport to Schwab with this form. You may need to obtain an Individual Taxpayer
Identification Number (ITIN) from the IRS before you can complete IRS Form W-8BEN. Please visit the IRS website at
www.irs.gov for more information.
• Schwab Global Accounts: If the decedent had a Schwab Global Account
™
associated with a Schwab One
®
account, the
beneficiary(ies) must meet Schwab Global Account eligibility requirements to maintain the assets in kind. If the
beneficiary(ies) does not meet the requirements, Schwab may close the Schwab Global Account. In closing the account,
Schwab will transfer all eligible securities to the decedent’s Schwab One account and sell ineligible securities in the
Schwab Global Account, convert all foreign currency into U.S. dollars, and transfer the amount to the decedent’s
Schwab One account. Securities will be valued in U.S. dollars in the decedent’s Schwab One account. Foreign currency
transfers will be subject to exchange charges.
• If you are requesting a lump-sum distribution:
– A new Schwab One account must be opened so assets can be registered in your name
– Once the assets have been transferred into the account, your distribution will be processed*
– Please call 1-888-297-0244 with any questions

assets into the new account. (Attach a completed Schwab Bank High Yield Investor Checking Account and Linked
Schwab One Brokerage Account Application.)
Open a new Schwab One custodial account in the name of the minor beneficiary and transfer the assets into the
new account. (Attach a completed Schwab One Custodial Account Application.)
Sell all my securities and send a check.*
*Note: If you elect to sell securities, please note that sales of existing positions will not be completed until the new
account has been opened. Since the prices of securities may fluctuate, the value of the existing positions may rise
or fall between the time this Application is completed and the time the existing positions are sold. You agree that
