Safe Deposit Box – Property Release Form
Unclaimed Property safe deposit box contents can only be mailed to one owner or heir if there are
multiple owners or heirs. Therefore, all other owners or heirs must sign and submit a Property Release
Form, authorizing release of the property to one owner or heir, along with a Claim Affirmation Form.
Both forms can be found on our website at: http://www.sco.ca.gov/upd_form_claim.html.
Each owner or heir Claim Affirmation Form must be signed, notarized and accompanied by a copy of
their driver license or state issued photo identification and proof of their Social Security number. Heirs
must provide additional documentation showing their relationship to the deceased owner proving they
are entitled to the property. Please see our Deceased Owner Filing Instructions located on our website
