This form is not a waiver of minimum requirements for pre-construction treatment. It is
intended for use only after a portion(s) of a treatment has been unintentionally omitted.
Statement of buyer:
I (we) the undersigned buyer(s) of the above property, understand that the slab areas
(described below) of the property were not treated for the prevention of the subterranean
termites during its construction. I (we) further understand that such treatment is required
under minimum standards for the pretreatment of structures under rules adopted by the North
Carolina Structural Pest Control Committee and that I (we) have the option of having this
treatment performed at this time.
I (we) do not wish to have this work performed and do hereby release the below named
company from drilling the slab areas and treating the soil beneath the slabs.

The following slab areas of the above structure were not treated for the prevention of subterranean
