SECOND DEGREE APPEAL
(2021-2022 Academic Year)
STEP 1: Read the CCC Satisfactory Academic Progress (SAP) Policy
I have read the Satisfactory Academic Progress Policy.
STEP 2: Complete and check off requirements for a Second degree:
I am pursuing a program past my first degree at CCC in and have declared this
major with Registration and Enrollment Services.
I understand that I must have all official academic transcripts evaluated and applicable hours transferred to CCC by the
Office of the Registrar prior to this appeal being reviewed.
I understand that I will be funded one time for the courses required for the second degree. I will not be funded for them
a second time if I do not pass or complete them.
I understand that I am responsible for enrolling in the courses required for my Second degree as listed on
DegreeWorks, available through MyCCC account.
I understand I will not be funded for courses not required for my Second degree at CCC.
I understand that if approved, I will not be funded to repeat courses previously taken at CCC or transferred in unless
approved by Director or Dean of the program for my second degree.
I understand that if I change from my approved second degree program, I will no longer be eligible for funding.
I understand that if I already have two or more degrees (Associates, Bachelors, Masters or Doctoral) in any
combination, this appeal will be denied.

enroll in CCC’s payment plan at www.coconino.edu/cashier to prevent my classes from being dropped. Submission of this appeal does
not guarantee approval.
