4. The Proposed Ward is an incompetent or mentally incompetent person who is unable,
unassisted, to properly manage and take care of himself/herself or his/her property;

The Proposed Ward filed a voluntary petition for guardianship.
5. The appointment of a guardian is necessary for the following reasons:

6. The requested guardianship is the least restrictive and most appropriate guardianship
suitable to the Ward’s circumstances and needs.
7. The Proposed Guardian is a fit and proper person to serve as guardian for the Ward.
8. The Proposed Guardian has priority for appointment, or those who have a higher priority
are unwilling, unable, or unfit to serve as guardian.
9. It is in the Ward’s best interest that the requested guardianship be approved.
10. The Guardian shall not be required to give bond.
