2020-2021 DEPENDENT STUDENT: VERIFICATION OF RECEIPT OF SNAP BENEFITS
Your 2020–2021 Free Application for Federal Student Aid (FAFSA) was selected for review in a process called verification. The
law says that before awarding Federal Student Aid, we may ask you to confirm the information you reported on your FAFSA. To
verify that you provided correct information, we will require certain information to be provided to our office. You must complete
and sign this worksheet, and submit the form and other required documents to the Financial aid Office. We may ask for additional
information. If you have questions about verification, contact the Financial Aid Office as soon as possible so that your financial aid
will not be delayed.

The student, or the student’s parents’ household, did not receive SNAP benefits during 2018 or 2019.
The student, or members of your parents’ household, received benefits from the Supplemental Nutrition Assistance Program or
SNAP (formerly known as food stamps) any time during 2018 or 2019. If asked by the student’s school, documentation from
the agency that issued the SNAP benefits will be provided.
The parents’ household includes:
a) The student.
b) The parents (including a stepparent) even if the student doesn’t live with the parents.
c) The parents’ other children if the parents will provide more than half of their support from July 1, 2020, through June 30,
2021 or if the other children would be required to provide parental information if they completed a 2020-2021 FAFSA.
d) Other people if they now live with the parents and the parents provide more than half of their support and will continue to
provide more than half of their support through June 30, 2021.
