SOUND RECORDING PRODUCTION TAX CREDIT APPLICATION
I hereby certify that the sound recording production tax credit applicant identified in this application has no liability to the State of
Louisiana and has never declared bankruptcy in accordance with R.S. 47:6023 and applicable rules.
REQUIRED ADDITIONAL INFORMATION (See next page)
1. An application fee of $250. Projects under $15,000 are not subject to the fee.
2. A deposit shall be remitted to Louisiana Entertainment for the verification report fee, based on production size. Prior to the issuance of
the initial certification letter, an expenditure verification report deposit shall be submitted accordingly to the following schedule.
a. Production Size Deposit Amount
$10,000 - $24,999.99 $750 (Maximum fee $1,500)
$25,000 - $49,999.99 $1,500 (Maximum fee $3,000)
$50,000 - $99,999.99 $2,500 (Maximum fee $5,000)
$100,000 – Above $3,750 (Maximum fee $7,500)
b. Please submit two separate checks for each fee with the designation noted in the memo line. Please mail to the attention
of Louisiana Entertainment at 617 North Third Street, 11th Floor, BR, La 70802 or P.O. Box 94185, Baton Rouge, LA 70804-9185
3. Detailed distribution plan.
4. Detailed preliminary budget.
5. Brief description of the sound recording production.
6. If this application is for a series of productions, the title, budget and a brief description must be provided for each production in the series.
I hereby certify that the information provided in this application is true and correct, and I am aware that my submission of any false information
or omission of any pertinent information resulting in the false representation of a material fact may subject me to civil and/or criminal penalties
for filing of false public records and/or forfeiture of any tax credits approved under this program. I also understand that any final certification
of investor credits will be subject to the submission and review of an independent cost report and/or audit conducted by a Certified Public
Accountant licensed in the state of Louisiana. I understand that this application and information submitted with it shall not be returnable
to the applicant. As a condition for receiving certification of tax credits, state-certified productions may be required to display the state
brand and/or logo.
All fees must be paid in full before the application will be processed. Fees can be paid by check made payable to Louisiana Economic Development.
