Declaration: I understand approval of this request means the course will remain on my permanent academic record with
the notation “LWD” (late withdrawal). This course notation will have no effect on my GPA. I further understand
courses listed as “LWD” do not result in any reversal or refund of tuition fees and may result in a reduced course load,
which may render me ineligible for scholarships and the Dean’s Honour List. Additionally, it may affect my eligibility
to enrol in subsequent courses, and, ultimately, the timing of my graduation and/or eligibility to graduate with honours.
