Failure to pay an installment from a valid repayment plan can be cause for eviction.

1. In Column A, enter the date that rent and/or utilities were due for each rental period that began between March 18, 2020
and August 17, 2020. For example, if the tenancy agreement specifies that rent and/or utilities was due on the 1st of the
month, the first rental period would start on April 01, 2020, and the last period would start on August 01, 2020.
2. In Column B, enter the amount of rent and/or utilities that was due for the rental period.
3. In Column C, enter the amount of rent and/or utilities that was paid for the rental period.
4. In line 13 enter the total amount from Column B.
5. In line 14 enter the total amount from Column C.
6. In line 15 calculate the amount of arrears owing. Enter this number in line 16 on the next page.
