CAMPER REGISTRATION FORM
This registration form and payment must be submitted to UDCR concurrently. Payment must be made in full at the
time of registration to ensure space in camp. Each camper must be enrolled by the registration deadline for each
week of camp. Form(s) can be submitted in-person at the RecPlex, or via mail, fax or email.
MINORS PAPERWORK — Is collected through a system called CampDoc. Further instruction will be sent out via email upon
completion of registration.
CAMPER AGE — Camper’s birthday must fall between 5/30/2006 and 9/30/2013.
UD RATE — The UD rate applies to current students, faculty/sta, and UDCR members.
SIBLING RATE — Full price for the rst child; $15 o every additional sibling registration within the same week of camp.
EARLY DROP-OFF/LATE PICK-UP — To help accommodate schedules, campers may register for early drop-o prior to
the start of camp, between 7:30am–8:00am, or stay for late pick-up after the camp day ends, between 5:00pm–5:30pm.
Extended care is an additional cost for each camper and must be registered for the week in advance.
