RationaleforProposedForm
ThisformfollowstheIRBmodelforprotocolsubmissions.Inthismodel,therearetwoformsfor
theprotocolsubmissionbasedoncriteriaofperceivedlevelofrisk.IRBthenreviewsthe
submissioninordertodeterminewhethertheprotocoldoes,infact,fititscategoriesfor
expeditedorfullIRBboardreview.WhiletheIRBhassetstandardsforeachtypeofproposal,
thereisa“fudge”factorcenteringonthequestionofwhetherthereistrulyminimalrisk.
Theexampleherecentersontheconceptof“substantialchange.”Aprogramordepartment
wouldsubmittheirproposal,answeringthequestionsregardingthenatureofthechanges,and
theOfficeof(GraduateorUndergraduate)Educationwouldmakethefinaldeterminationasto
whethertheproposalisjustforinformationalpurposesorrequiredafullcommitteereviewand
vote.Thesignatureswouldonlybeontheinitialsubmission.Suchproposalswouldnothaveto
besentbacktocollegesordepartmentsforreapproval.TheAVPfor(Graduateor
Undergraduate)Educationcanmakeadeterminationbasedonpresentedinformationaboutthe
natureoftheproposedchanges.

feelisminimal,butthereviewprocessindicatesthatitismorethanminimal(thusrequiringfull
reviewandvote).Aprogrammightsubmitachangethattheyviewassubstantialandthereview
processfindsittobeonlyminimal(thusrequiringitbesubmittedforinformationalpurposes
only–withinformationgoingtotheappropriatesourceslikecatalog,registrar,etc.).
Bycreatingthisformandhavingitavailabletwoweeksbeforeanymeeting,thecommitteecan
haveastandardizedmannerinwhichinformationispresented.
