1. Why do you want to complete the Phlebotomy program? (Please be specific)
2. What do you hope to gain most from completing this program (not just professionally but also personally)?
3. Give your personal interpretation of medical professionalism, and how you can best exemplify this concept. Use
a real-life example if possible.
4. State three examples of how you personally have displayed professionalism in a past job, volunteering or school
experience. (Note: Telling me that you “acted like a professional” does not tell me what actions a professional
would take in a given situation, so please describe the traits you displayed that make up acting like a
