Recruitment Documentation Checklist
The following documentation must be brought with you at a later date identified, you will
need to provide a copy of each document as well as the original for validation.
College Diploma (Or a letter from college confirming graduation date)
AEMCA Certificate (Or letter of registration with date to write AEMCA testing)
Current CPRBLS or Health Care Provider
Driver’s Abstract 3 year search issued within the last 60 days.
Clear color copy of Class F Driver’s License (front and back)
Successful Physical Agility Evaluation (in last 6 months) At this time we are only
accepting OPPAT testing. Please book your test appointment at www.OPPAT.ca
NOTE: Successful completion of OPPAT must be obtained prior to being
invited to commence the face to face interview phase of the recruitment process
(STAGE 4).
Copy of any relevant certifications (i.e. Base Hospital certifications, BTLS, etc.)
Criminal Record Check (ensure this check includes a reference to your ability to
work with the vulnerable sector) Issued within the last 90 Days.
Proof of Immunization signed by a physician for the following:
Measles, Mumps, Rubella
Diphtheria, Polio,
Chicken Pox,
Hepatitis B
Tetanus (issued within last ten years)
Influenza
Proof of COVID-19 Vaccinations
3 Written Letters of Reference
Preference will be given to candidates with the following skills; please provide proof of
any:
12 lead ECG certification with STEMI recognition training
PCP autonomous Intravenous certification
Continuous positive airway pressure (CPAP) certification
Supraglottic (iGel) airway certification
