Definition: Use this form for requesting approval to create a new position or to hire a new employee.
Supervisor (or Department Manager) drafts new or revised job description and letter explaining why the position is being created or changed when
necessary. Recommend discussion with VP/Dean/Provost prior to completion of form and assure funding is in current year budget.
Supervisor (or Department Manager) completes all items for requesting a new or replacement position and all applicable items in Section A
and Section B of this form and submits with Step 1 documents to Human Resources for review and classification.
