3. In 1-2 typed pages, give us a personal statement describing any relevant information
about yourself, your educational and professional goals. This should include a
description of your community involvement and where you are planning to attend in the
fall.

The scholarship application packet must be received, at 43460 Ridge Park Drive, Suite 200,
on or before 4:00 PM on Monday February 25, 2019. Any application received after this time,
will NOT qualify. We are not responsible for late or undelivered mail. You can drop off the
application packet at the front desk. Or, you can fax the application, but we would recommend
you call to be sure the fax has been received in its entirety. Unfortunately, applications cannot be
submitted via email at this time.
The Committee may request a personal interview to assist in making their final determinations.
Finalists will be notified by telephone or email by March 19, 2019. If you are chosen for our
scholarship, we will award it at our PWR luncheon on Thursday, April 4, 2019. The luncheon
will begin at 11:15 a.m. and conclude at 1:00 p.m. You should plan to be there by 11:00 and stay
until about 1:30 for pictures and mentor assignment (mentor assignment is optional).
