PETITION FOR READMISSION FOLLOWING ACADEMIC SUSPENSION
A student who has been suspended from Honolulu Community College may petition for readmission after
having “sat out” at least one semester from Honolulu CC. To complete this form, fill out the
information requested below, then meet with an Academic Counselor.
I
f the suspended student did not change his/her home institution, but continuously took classes at another
UH community college, this completed form must be submitted to the Admissions Office by August 1 for
the Fall semester and December 1 for the Spring semester.
I
f the suspended student changed his/her home institution or stopped attending classes within the UH
system, in addition to this completed form, a system application by August 1 for the Fall semester and
December 1 for the Spring semester.
