I request Pass/No Pass grading for the following course:

PLEASE READ PRIOR TO SUBMISSION!
I understand and agree to all of the following:
1. A student may elect ONE course per semester to be graded on a Pass/No Pass basis.
Once elected, the Pass/No Pass grade may not be changed to the letter grade
(A,B,C,D,F) assigned by the instructor.
2. Selecting this grading option may affect the transferability of the course to a college or
university. Students are advised to consult with a counselor for current policies
regarding Pass/No Pass grading before submitting a petition for this option.
3. Grades of A, B, and C will be recorded on my permanent record as P (Pass).
4. Grades of D and F will be recorded on my permanent record as NP (No Pass).
5. Units attempted for this option are not computed in the grade point average, but are
used for determining Progress Probation and Progress Dismissal. Financial Aid eligibility
may be affected.
6. A maximum of 15.0 units under this grading option may be applied toward the
Associate in Arts or Science degree.
7. Petitions for this option must be filed with the Admissions Office by the published
deadline dates in the FLC Class Schedule or online.
