PLEASE LIST THE NAMES OF ALL PEOPLE WHO WILL USE THE TENNIS COURT WITH YOU:
*A Hold Harmless Form is required for EACH individual (including yourself) who will be on the tennis courts, whether he/she is
playing or just watching. If you are with a company providing insurance for use of the tennis courts, Hold Harmless forms and a list of
people attending is not needed.

Tennis Court Rules
1. Rentals are for 1 hour of use on each day specified (unless otherwise agreed upon). Those who stay longer than 1 hour
may be asked to leave by Campus Police or have their permit revoked.
2. Tennis Court priority ALWAYS goes to PCC classes. The courts are NOT available for use when a tennis class is in session,
even if the class is not using every court.
3. Tennis Courts cannot be used for the purpose of generating income.
4. Other players not listed on this application may be allowed to play as long as he/she brings a signed Hold Harmless Form
with him/her.
5. Please call Campus Police at 626-585-7484 if the gate is locked when you arrive. The tennis courts will only be unlocked
for the person on the permit. Please bring a copy of your permit and ID every time.
6. If you need the lights turned on, please call Campus Police at 626-585-7484.
7. Payment for your tennis court use is due in advance. If payment is not received, your reservation may not be honored.
8. There are no refunds for reserved tennis court time that you do not use.
9. Restroom facilities are only available during normal business hours.
10. Parking is available in open lots. Appropriate parking fees should be paid upon arrival every day. Daily permits may be
purchased from dispensers located in every lot. Permits are $2 and can be paid with cash, credit or debit card. Tickets will
be issued for all parking violations.
11. Please do not bounce balls against building walls, loiter or sit in the garden beds. Please use receptacles provided for trash.
