Equipment Use Request Form
The Office of Student Life & Leadership has equipment that can be checked out for use
by Palomar
College Departments or Palomar College Clubs. Please complete this form and email it to the Student
Activities Coordinator, Kelly Donaghy at kdonaghy@palomar.edu
Requests are not guaranteed until you receive a confirmation email from the Office of Student Life &
Leadership. All equipment must remain on Palomar College campus property.
Any repairs to equipment which are deemed necessary upon return, will be the responsibility of the
department/club/ASG that used the equipment.
