ou may claim the Lott
ery and Gaming credit for 2020 property taxes payable in 2021, if, on January 1, 2020, you were
the owner of the property listed above and used that property as your primary residence. Your primary residence is where you
live most of the time. When you are temporarily away, it is the home to which you intend to return. You can have only one
primary residence and claim only one lottery and gaming credit. You cannot claim this credit on business property, rental units,
land, garages or other properties that are not your primary residence. A non-WIsconsin resident cannot claim this credit. If
you meet these requirements, you may claim the credit using the form below.
To establish your eligibility for the credit, you must provide proof of residency. Copies of the following are acceptable proofs:
1. A current and valid Wisconsin Driver’s License.
2. A current and valid Wisconsin Identification Card.
3. A gas, electric, or telephone service statement (utility bill) for the period that includes January 1, 2020.
4. A

Sign the claim form where indicated, date it, and print your name clearly below your signature.
Only one property owner is required to sign the claim form. All claims for the 2020 credit
must be received
by the City Treasurer’s Office by January 31, 2021.

f you do not qualify for the credit. The Wisconsin Department of Revenue will be
auditing lottery and gaming credit claims. Note that Wisconsin State Statutes require that you notify the
City Treasurer within 30 days after you no longer qualify for the credit because you no longer own the
property, or no longer use it as your primary residence.
