to complete your financial aid file.
Verification is a process required by the U. S. Department of Education to review the accuracy of information
reported on the Free Application for Federal Student Aid (FAFSA). Further processing of your financial aid
will not occur until the information requested below has been received by the Office of Financial Aid.
DIRECTIONS: Complete and return this form with a copy of one of the following documents that
indicate your high school completion status when you will begin college in 2019-2020.
Check which document you are attaching to this form. Sign, date and return this form with a copy of
the documentation to the Office of Financial Aid at Purdue University Northwest.
A copy of the student’s high school diploma.
A copy of the student’s final official high school transcript that shows the date when the diploma was
awarded.
A copy of the student’s General Educational Development (GED) certificate or GED transcript.

If State law does not require a homeschooled student to obtain a secondary school completion credential

equivalent, signed by the student’s parent or guardian, that lists the secondary school courses the
student completed and documents the successful completion of a secondary school education in a
homeschool setting.
