Lead Student Observation and Feedback Sheet
Not only does the lead student facilitate the gathering, but he or she follows the guidelines in the Gathering
Guide to model the role of an active learner. Please complete the following form every week for each lead
student. At the end of the gathering, spend five minutes reviewing your notes with the lead student, and provide
him/her this form.
PREPARED: The Lead Student was prepared to facilitate the lesson (professionally dressed, organized and ready,
with lesson materials such as handouts, examples on board, etc.).
