school, college or university, Doctor, Nurse, Dentist,
Social Worker, Certified Solicitor, a bank or building

DWO Advisor, your Prison or Probation Officer

Passport-sized photographs
You will need two good quality colour identical head and
shoulder passport sized photographs with a plain, pale
background. Your head must be in the middle of the
photograph. Poor quality images e.g. self-printed photos will
always be rejected, delaying your application!

Card application costs £15, including Recorded
Delivery. Your card will be issued usually within
10 days of application
Urgent Application* - If your application is urgent
we offer a rapid-service costing £25.00, including
