• This form must be submitted to the campus Admissions Office by 5 pm on the last day of add/drop week for the
earliest part of tem in which you are currently enrolled.
• If you are changing your major/program, your catalog year will also be updated to the effective term.
• If your major is AA/GENR and you are adding or changing a pathway, this will not extend your financial aid eligibility.
Only courses required for the AA/GENR (not a specific pathway) are used when determining enrollment levels for
financial aid.
