Coal Run is impaired for fecal colliform.

FSU has an animal waste program on campus. While animals are not allowed on inner campus &

athletic fields, we do house a veterinary technology program in Hunt Haught Hall. Our three denoted

hot spots have greatly improved. People have started to use the waste stations. Our pet waste

brochure is distributed to the Veterinary Tech program and at special events.

This years dry weather inspections have not shown any additional sewer leaks. The City of Fairmont

did well to correct them all the previous year.

FSU continues to work with various student organizations to participate in litter clean up initiatives.

Fairmont State has continued to develop its partnership with the City of Fairmont. We have

successfully implemented FSU's first bioswale with the Cities help.

Due to budget cuts our partnership with Prunty Town correctional institute was voided and they are no

longer assisting with litter cleanup.

Fairmont State continues to schedule events with "Make Marion Co. Shine" a community organization.

This year we branched out to the WVDOH and met some very good contacts to discuss and swoop

ideas.

Update: FSU began working with the Housing department to create a community service program

for students who get into trouble to assist with litter cleanup. This has been very successful. In fact

we have had students come to us looking for community service opportunities for scholarships and

other incentives. Hopefully this new program will continue to grow to replace our adopt a parking lot

initiative.

Our website provides information on MS4, FSU's SWMP and web links to fact sheets. The website has a

built in counter (61 views last year) and allows people to submit feedback. Goal to revamp website with IT

and Graphic departments help in 2016. Want to make more interactive and fun to get more attention.

FSU continues using mass email communications, posters, and weekly online updates.

All freshman in Fall 2015 received a stormwater educational brochure as part of their orientation packet. We

also regularly stock these brochures in 3 buildings and track the number taken weekly.

We continue to check our advertising posters (one per building) and replace as needed.

Physical Plant staff yearly training sessions continue to provide information on the SWMP, IDDE Policy,

E&SC policy, PPP, and our spill cleanup procedures.

Our website visits were tracked with the assistance of IT. In 2015 our hits decreased by 72.

Twenty-Five (25) stormwater brochures have been taken since January 2015 showing there are

interested students that are aware of stormwater pollution.

Seven hundred stormwater brochures were provided to new freshmen during orientation. (500 FSU,

200 PCTC) Another 51 stormwater brochures were provided at special presentations or events.

Some include: Kick Butts Day (cigarette litter), and Engineering Society Quarterly Meetings.

One poster is in each building in a conspicuous place. Three were replaced over the year because

they were torn, missing, or faded.

During physical plant trainings, staff read through the document, watched brief videos, have hands

on training when able and finish by taking a short quiz to ensure they comprehended the material.

Anytime we identify an opportunity to educate students/faculty/staff we take that opportunity. Sometimes these

in-prompt conversations are the most rewarding. For the most part these have been recorded in a public

education/outreach log.

In addition, Stephanie Slaubaugh has attended several professional meetings as the guest speaker. Some topics

presented were MS4s, storwater pollution, what FSU is doing, and bioswales.

Also, Stephanie spoke to the FSU faculty/staff population at the Fall 2015 welcome back ceremony. She provided

a general overview of FSU's program and had interest from several professors who will be good assests.

The annual campus clean-up initiatives have been very productive. We even have been partnering

with "make Marion shine" community group and have had additional cleanups. We've even had

community memeber show up to participate in our campus cleanups.

Update: The Student Mis-Conduct community service and other community service opportunities for

students has been much more productive than our "adopt a parking lot" from 2013-2014.

Weekly updates are provided to the President and her cabinet members including MS4 related

events and trainings. The presidents cabinet continues to be a good resource in reaching out to

Deans and faculty. In addition, Stephanie has begun providing MS4 updates at all Board of

Governor meetings.

We also have an online form that can be used to provide input and submits directly to the physical

plant.

Any and all student/faculty/staff/community participation has been recorded.

FSU's SWMP is provided online for anyone to access and comment.

Continued supply of information to the Board of Governors, President, Cabinet members, and Deans

with the MS4 program.

We have also worked with the marketing department and social media to help spread awareness.

organizations along our MS4 journey.

The MS4 map was updated by performing some in-house dye testing/ smoke testing of storm drains

to confirm or deny their routing. Where discrepancies were found the CAD utility drawing was

updated to reflect those discoveries.

FSU has divided campus into multiple quadrants to perform future investigation including possible

cameraing of lines to check pipe conditions. Several engineering firms have this information and are

beginning to work on price quotes.

Eight illicit discharges were observed during inspections. These consisted of oil leaking from

vehicles on campus, small amounts of dripped gas at refilling station, small grease spill. One illegal

dumping occured, someone entered the physical plant premises during the night a dumped a

container or oil in our compactor, it was a mess to clean up, and we are now locking the rear gate.

Oil is by far our largest concern. Then grease and gas.

FSU has educated our community of our efforts and what to do when a spill is spotted and how to

report.

IDDE training was provided to all Physical Plant staff and custodian workers. Including what to look

for, proper cleanup and how to report. These steps have now been written down in a spill response

plan. Also covers our workers safety.

We also have trained our staff on our PPP and good housekeeping efforts, this helps to eliminate

potential discharge concerns.

A short quiz was created and administered after review of the policy and what to do if an illicit

discharge was detected. Also began hands on training with our spill kits so staff know how to

properly use.

This training was also evaluated by the quick response time for reporting and correcting the illicit

discharges we had this last year.

FSU continues to be proactive as opposed to reactive. Many of our illicit discharges were very small

amounts but were good learning opportunities for staff.

Twenty eight employees recieved training on our IDDE policy, E&S Policy and PPP. An hour long

training was provided to all employees. A copy of the IDDE policy was provided to all employees. A

review of the policy was conducted as well as what to look for and how to report. A short video was

also shown reviewing different illicit discharges, what to look for, and proper ways to

correct/investigate. Hands on training was provided with our actual spill kits that are in each state

vehicle. A brief quiz was administered at the end of the training as well as a Q&A session.

Through the amount of people who participated and the knowledge retained for the quiz.

One new project. It is an apartment project to provide student housing on campus. The project is a

combination of apartments and dorm suites. It involves taking a large parking lot and creating three

buildings in a U-shape with an inner grass field.

We only have one construction project this year and it is ongoing. The existing site was a 210 space

parking lot and now has be re-purposed for campus housing with a large green quad and other

landscape features. This project qualified for development incentives but they were not used. The

design is around 1" capture of rainfall, no deductions were taken.

27 inspections were conducted by FSU personnel. As part of the contract, the contractor is

responsible for stormwater inspections, every 7 days or after a rain event. This began from the start

of construction, May 2015. Since May, FSU has had to request maintenance or replacement of

BMP's five times. The contractor has been extremely proactive and corrected any deficiencies within

24 hours of FSU notifying them. All deficiencies have been brought to compliance to date. The most

common issues are silt fence and inlet bag maintenance.

All Physical Plant employees received the training this year.

Other departments such as Roads & Grounds have reviewed multiple times with their immediate

supervisor. All trainings were recorded in our log and effectiveness of their better housekeeping

skills shows in our inspection reports.
