MED TERM FACULTY SCHOLARSHIP
The $250 Med Term Faculty Scholarship is awarded to qualified students enrolled in Medical Terminology at
Moraine Valley Community College. This scholarship application is for students enrolled in the fall 2020
semester.
Please return this completed application to the Moraine Valley Foundation office by emailing an attached
copy to friendp5@morainevalley.edu or mailing it to Moraine Valley Foundation, 9000 College Parkway,
Palos Hills, IL 60565 attn. Med Term Scholarship by 5 p.m., Tuesday, September 15, 2020.
INSTRUCTIONS: Use Adobe Acrobat to read the file. If prompted, click "Open With Different Viewer"
button. Fill out the form. Then, go to the "File" menu and select "Save As." Name the document and save
it to your computer. Finally, open a new email in your email program, fill in the to address and subject
lines, attach the pdf you saved earlier to the email and send. If you are having problems attaching a pdf,
you could take screenshots of the completed application and email the photos.
