Optional Checklist to Apply for the Filipino World War II

Checklist
This is an optional checklist to help petitioners who want to submit Form I-131, Application for Travel
Document, under the FWVP Program.
Do not submit this checklist to USCIS.
I read the Program Eligibility section and reviewed the FWVP Program Eligibility chart under the
Program Eligibility section on the FWVP website at www.uscis.gov/FWVP to verify my eligibility for
the FWVP Program.
I completed Form I-131, Application for Travel Document, for each relative:
I typed or printed “FWVP” at the top of the first page of Form I-131.
I entered complete information about myself (the petitioner or self-petitioner) in Part 1., including my
address.
I selected Part 2., Item Number 1.f. (“I am applying for an Advance Parole Document for a person
who is outside the United States”).
I entered complete information about my relative (the beneficiary) in Part 2., including my
beneficiary’s address. If I am a self-petitioner, I provided information about myself or my derivative
beneficiaries.
I understand that I should not complete Parts 5. and 6., which do not apply to the FWVP Program.
I fully completed Part 7.
I signed and dated the form in Part 8. and included my daytime telephone number.
If applicable, the person who helped me prepare Form I-131 has completed and signed Part 9.
I included all requested documentation, including any documentation of recognized military service and
evidence of a qualifying relationship with the veteran that existed on or before May 9, 2016. If I cannot
establish military service, I understand that USCIS will review any of my prior military service records
that are available.
If available, I included a photocopy of my Form I-130 approval notice (Form I-797, Notice of Action) or
a printout from Case Status Online which shows the approval of my Form I-130. I understand that if my
Form I-130 is pending, I may call the National Customer Service Center to request expedited processing
of my Form I-130. I can find more information under Requirements for Requesting Parole.
If the Form I-130 petitioner has died, I included evidence that my Form I-130 was reinstated by USCIS
or, if not already reinstated, I have included either a request for humanitarian reinstatement or an
explanation that my request for humanitarian reinstatement is currently pending with USCIS.
I included the fees for each Form I-131 that I am filing. If I am eligible for a fee waiver, I also submitted
Form I-912, Request for Fee Waiver, with supporting documentation.
