2019-2020 Loan Proration Form
Last name First name NSCC ID number
You indicated on your Student Data Form that you will be attending less than two full-time
semesters during the 2019-2020 academic year (Summer 2019, Fall 2019 and Spring 2020). If this
information was reported incorrectly, please contact the Financial Aid Office. You must complete
and return this form to be considered for student loans.
Federal regulations require the maximum annual Federal Stafford Loan amount an undergraduate
student may borrow to be prorated in cases where it is known that the student’s remaining period of
study will consist of less than one (1) Academic Year.
The prorated amount the student may borrow in this period of study is based on a ratio of the
number of credits enrolled divided by 24. This amount is then multiplied by the student’s annual
loan limit (based on dependency status and grade level) to determine the maximum the student
may borrow in their final terms of undergraduate study. Any semester with less than 6 credit
hours will not be included in proration.
Loan eligibility will be prorated based on enrollment after drop/add period (first week of class).
List below every course you will be enrolled in during the 2019-2020 academic year.
Student Certification: I certify the information on this form is accurate and complete to the best of my
knowledge. I understand any false information will be cause for denial, reduction or repayment of financial
