I hereby request payment of my University Scholarship for the 2020 Summer Quarter and I hereby certify:
(Eligible scholarships include National Merit, Presidential, State Board of Supervisors, Deans, Valedictorian, and Outstanding Student.)
My University Scholarship amount will be pro-rated based on the number of hours enrolled.
I understand that if a University Scholarship payment is made on my behalf for the 2020 Summer Quarter.
I must have attended Louisiana Tech as a scholarship recipient during the previous academic year.
I understand that to be eligible for payment during the 2020 Summer Quarter:
I must be eligible to retain my scholarship(s) based on my GPA and have earned at least 24 hours for the academic
year at the end of the Spring Quarter.
My remaining University Scholarship eligibility will be reduced by one quarter.
-National Merit and Presidential Scholars (awarded prior to 2017-2018) may receive their full scholarship award during the
Summer Quarter ONLY if they enroll full-time (8 or more credit hours). National Merit and Presidential Scholars who enroll in 6
or 7 credit hours during the summer will be limited to the lesser value of the State Board of Supervisors' level of award, or the

Scholarship recipients who enroll in 6-7 hours will receive 75% of their regular quarterly scholarship amount.

I must have attended Louisiana Tech as a scholarship recipient during the previous academic year.
I understand that to be eligible for payment during the 2020 Summer Quarter:
I must be eligible to retain my scholarship(s) based on my GPA and hours earned at the end of the Spring Quarter.
I must enroll in and pay tuition/fees for a minimum of 8 credit hours in the 2020 Summer Quarter, and all other
terms and conditions of my original contract apply.
I certify that I have read the Summer Scholarship eligibility criteria. If students are taking one or more courses
that begin prior to July 1st, the application deadline is May 27th; if students are only taking courses that begin
after July 1st, the application deadline is July 8th.
