By completing this election form and submitting it to the Clerk’s Office, I am confirming that I have read the
Defensive Driving Course Information section below. I am electing to pay the assessed fine and attend a Defensive
Driving Course to avoid points on my driver history.
I also understand that I must PRESENT PROOF OF COMPLIANCE to the Clerk of the Court within 60 days of the
date this election is received by the Clerk. By electing to attend a Defensive Driving Course, I understand that I
must complete the course and present my proof of completion to the Clerk’s Office, or else pay additional fees and
points will be assessed on my record.

Defensive Driving Course Information
- I understand this option is available to me if:
• I have
not
made an election within the last twelve months.
• I have
not
made five elections in a lifetime.
• I am making this election within
30 calendar days from the date the ticket was issued
.
• I have
not
been charged with a driver license, registration, insurance, non-moving violation or mandatory
court violation.
• I do not possess a Commercial Driver’s License
.
I understand to satisfactorily complete this election I must:
• Attend and complete a State approved defensive driving course
in the State of Florida
.

of Highway Safety and Motor Vehicles site at:
www.flhsmv.gov/ddl/bdis.html.
To locate an approved on-line basic driver improvement course, please use your web browser to
search the internet. Be sure the course is an approved course.
I understand that if I satisfactorily complete this election:
• My driver record will not reflect a conviction.
• Points will not be assessed against my driver license.
I understand that if I fail to complete any requirement of this election or I am ineligible for this election:
• I will be guilty of the offense and my driver record will be changed to show a conviction of this offense.
• Points will be assessed against my driver license.
• Additional fines, costs and fees will be due.
