Special Instructions:
Site General Manager is your ONLY POC for scheduling (NOT the Asst
GM. or Kitchen Manager or like). The Site GM should also be contacted
for any on site service, but if unavailable, obtaining the first name, last
name & title of a MANAGER is required noting a GM is unavailable.
1. Schedule visit with store manager. Identify yourself as an affiliate partner of Commercial
Fire. Record the manager name and title with whom you made the appointment and the date
and time of the appointment:

Failure to provide schedule information may cause this order to be re-directed to another
service provider! If this repair cannot be scheduled to meet the deadline, Commercial Fire
MUST BE called within 48 hours and the Repair Dept advised of the issues.
Successful IVR call-in and call-out is an essential part of this work order. Failure to
successfully IVR will result in a 10% penalty on the entire amount of the invoice for
this work!
2. Check in upon your arrival at the store. Call Commercial Fire IVR using the phone and
tracking number above. If there are any issues using IVR, call Commercial Fire at 800-241
-1277.
3. Scope Of Work:
As per your estimate for a Fire Alarm repair to correct the deficiencies noted at this
location.
Repair Description/Instructions:
Corrective Action Required: The technician will provide and install (1) new Siemens
horn/strobe. The technician will then test the device to ensure complete repair.
This repair order must be scheduled directly with the location.
Please provide the scheduled date to Commercial Fire within 48 hours.
4. Additional Work: None beyond the repair described in the approved estimate. Any
additional work found while making the repair requires a written change order. You also have
this work order stamped with the store stamp and signed by a manager. Only the signature by
the Manager on Duty will be accepted. This order is not complete and the invoice will not be
paid without signature of the manager on duty.
5. Check out before departing from the store. Call Commercial Fire IVR using the phone and
tracking number above. Enter the Closing Check Out number: ____________________.
