Once you have met the requirements for admission to the INSTRUMENTAL JAZZ PERFORMANCE option
(normally after your fourth semester of residence), fill out this form completely and accurately. Please note that
you must also be in compliance with ALL Department policies and requirements (e.g. current with recital
attendance, in compliance with ensemble participation requirements). When you have met these requirements,
present this application form to your prospective studio teacher for approval. This form should then be given to
the Instrumental Jazz Performance option adviser. You also need to complete a Change of Major Request form and
have it signed by your adviser. Your adviser will verify the information you have provided and submit the forms
to the Department office. All forms are located on department website.
