Jackie Faris-Rees Student Leadership Award 2019
Chico State Alumna Jackie Faris-Rees was president of Associated Students at Chico State in the late
1970s, a member of the Chico Unified District school board, a participant in the Chamber of Commerce
and Butte Community Action Agency, and she dedicated her life to public service and education in many
other ways. She was committed to coaching and mentoring students and was a leading advocate for
Associated Students. Sadly, Jackie passed away from breast cancer in April 2000. This award was
established to honor Jackie and to continue her legacy of developing and acknowledging excellent
student leaders.
Award
1 recipient will receive $2,300
Eligibility
All CSU, Chico undergraduate students who have demonstrated significant and
outstanding leadership
on campus and in community will be considered.
Nomination
Guidelines
Nominations may be submitted by current CSU, Chico faculty, staff members, and
students. Please complete the nomination form and attach it to a
letter of nomination
that:
• Describes your nominee's outstanding leadership activities. Please be specific,
noting events, dates, organizations, the individual's contributions, and how they
aided the organization, event or situatio
n
• D
escribes why you feel your nominee deserves this acknowledgement
• Includes additional materials such as news
articles, other letters of support for the
nominee, or a list of the nominee's previous accomplishments, if relevant.
Selection Process
Nominations will be reviewed by a committee representing Student Affairs, Financial Aid
and Scholarship, and
University Advancement. Winners and nominators will be notified
by
March 20, 2019.
Presentation of
Award
The Jackie Faris-Rees Student Leadership Award will be presented at the Student
Leadership Awards on April 11, 2019.
Deadline
Nominations must be submitted no later than 5 p.m. on Wednesday, March 13 to:
