Instructions for Adding New Ofcers to MIS: Refer to the “Add New Officer” wizard under the “Chapter Admin” section in MIS for
assistance. You will have the choice to display a member’s personal information in the directory.
Instructions to Maintain Ofcer Information: Refer to the “Maintain Officer Information” wizard under the “Chapter Admin” section in MIS
for assistance. Turn off the “active” flag if the member is no longer an officer of the chapter. You can also maintain the web contact information that
will display in the officer directory. Note that the web contact information is separate from the actual member contact information such as home
address. This allows published contact information as an officer while maintaining the privacy of the member.
Please submit one copy of this form to National Headquarters, one copy to the Regional Secretary Treasurer, and one copy for
Chapter records.
