You have the option to appeal state aid renewal based on hardship conditions. To appeal your renewal status, please
complete this form in its entirety, and include supporting documentation. Full information on UIW’s State Funding

The student must maintain the following academic standards to remain eligible for the renewal of the Tuition
Equalization Grant (TEG):
- Maintai
n a cumulative GPA of 2.5
- Complete 75% of hours attempted for the prior academic year
- Complete a minimum of 24 hours the prior academic year
Appeal Statement & Supporting Documentation
Reason(s) that you were unable to maintain the required academic standards:
Experienced death of relative, or has been caretaker for ill or injured relative
Experienced severe injury or illness
Other extenuating circumstances
Explain in detail the issues affecting your academic performance, how you have resolved these issues, and what you are
doing to prevent them from recurring. Simply stating your need for financial aid is not grounds for approval of your
appeal. You must attach supporting documentation (i.e. medical bills, letter from third party, etc.) to corroborate
your statement.
