Please answer the following questions. If you answer yes to these questions and provide the required
affidavit, you may be eligible for in-state tuition pursuant to Illinois law (110 ILCS 675/20-88).
1. Did you reside with your parent(s) while attending a public or private high school in Illinois at
the time of graduation?

2. Did, or will, you graduate from a public or private high school or receive the equivalent of a high
school diploma in Illinois?

3. Did you attend school in Illinois for at least 3 years at the time of graduation from high school or
receiving the equivalent of a high school diploma?

Please note if the above criteria are all met, but you subsequently established a residence in another state
before enrolling at this University, you are not eligible for resident tuition under Illinois law (110 ILCS
675/20-88).
The below affidavit must be filled out, signed, and submitted to ISU Admissions in order to be considered
for in-state tuition.
