Search Completion Form
A.A. Form 3-75
The unit head is required to sign the Search Completion Form and forward it to the appropriate Dean or Director. After
that person has signed the form and indicated his/her approval or disapproval, the form is either forwarded to the Provost
or appropriate VP for sign-off or returned to the unit head for indicated revision. The Provost or VP will forward the form
to the Affirmative Action Director/Assistant Director who will sign and indicate his/her approval or disapproval.
The department or unit will receive written approval via email from the Affirmative Action Office prior to any offer
being made to a candidate.
