This application form is only for international students. An international student is NOT a U.S. citizen and NOT a
U.S. permanent resident (green card holder). U.S. citizens and permanent residents should apply online at
www.suny.edu/student.
To avoid any complications or delays with the enrollment process, please complete this application with
information that matches your educational records and personal passport. Please fill out the form, click "File--
