DRUG PROBATION ORDER
The Defendant’s probation is conditioned on the Defendant complying with the following conditions of his/her probation:
( ) Appear in Court whenever Ordered. The Defendant shall keep the Clerk of the Court informed of his/her current
resident/mailing address at all times.
( ) The Defendant shall not be arrested for and/or convicted for any further criminal offenses.
( ) The Defendant shall not possess, use or have any contact with illegal drugs or associate with any persons who sell,
furnish, possess or use illegal drugs. A positive urine screen will be considered as evidence of a violation of this
condition.
( ) The Defendant shall not frequent any business or establishment that serves alcohol as its principal source of income.
( ) The Defendant shall not consume or be in possession of any alcoholic beverage.
( ) The Defendant shall report to the Court Referral Officer immediately, attend and successfully complete the
recommended program.
( ) The Defendant shall submit to random drug screen monitoring at the request of the Judge, District Attorney, and/or
Court Referral Officer. Failure to appear for drug screen monitoring shall be a violation of the conditions of this
probation order.
( The Defendant shall attend and successfully complete any program the Court shall order the Defendant to attend as a

( ) The Defendant shall be responsible for all costs associated with drug screen monitoring and/or any program to
which the Defendant is referred by the Court, the Court Referral Officer and/or the District Attorney’s Office.
Failure of the Defendant to pay the costs associated with any programs and/or drug screen monitoring shall be
considered a violation of the conditions of this probation order.

The Defendant is hereby advised that the Court may, after notice to the Defendant and a hearing, at any time revoke
or modify the conditions of this probation or change the period of probation. The Probationer shall be subject to arrest for
violation of any condition of the probation herein granted. The Court may, after notice and a hearing, for cause, order the
original sentence executed. A copy of this Order has been delivered to the Probationer.
