Upon consideration of the Petition for Appointment of a Guardian of the Person and

and having conducted a hearing and determined that the said disabled person lacks sufficient
understanding or capacity to make or communicate responsible decisions concerning his/her
person and/or property, including provisions for health care, food, clothing, or shelter, solely as a
