How to Complete and Submit the
Thesis Adviser Selection and Agreement Form
• General notes:
o Your thesis adviser must be a permanent member of the faculty, have the title assistant
professor, associate professor, or professor, and have a doctoral degree (unless the master’s
degree is the terminal degree in your discipline).
o Visiting or adjunct faculty, instructors or lecturers may not serve as primary thesis advisers
(i.e. those who assign the grade for your thesis hours). In some circumstances, such
individuals may be approved by the Honors College to serve alongside a member of the
permanent faculty as co-advisers. Students seeking permission for this arrangement should
send a memo to the dean with a detailed explanation of the rationale for the request.
o Should you wish to have an adviser outside of your major, you must first obtain approval
from the dean of the Honors College. Fill out this form, send it to your proposed adviser and
submit the completed form to the dean.
o After completing the required fields (electronic completion required), collect appropriate
signatures. Note that electronic signatures are not acceptable. Submit the completed form by
the deadline stated in your HON 300 syllabus.
o Submission is not complete until you have followed all steps.
• Signatures Required:
If you have one thesis adviser and s/he is in your major, you need signatures from:
• The thesis adviser
• The school director
If you have co-advisers and both are in your major, you need signatures from:
• Both thesis advisers
• The school director
If you have co-advisers from different programs, you need signatures from:
• Both thesis advisers
• The directors of both schools
If you have been approved by the honors college dean to complete a thesis outside your major, you
need signatures from:
• The thesis adviser
• The director of your thesis adviser’s school
• The director of the school of your major
• Steps for Completion (complete in order):
1. Type the requested information into the respective fillable fields of the form.
2. Print out the form, sign and date.
3. Collect the appropriate signatures in the order they are listed above.
4. Scan the completed form and submit by uploading the pdf to your HON 300 ePortfolio.
