HYDRANT FLOW REQUEST
PAYMENT REQUIRED WITH APPLICATION (SEE FEE SCHEDULE FOR AMOUNT)
Must provide a map showing the locations of the two hydrants to be tested.
Acceptable methods are Google Maps, GIS or other similar maps. Hand drawn maps will not be
accepted. Street names, buildings, and any other pertinent information needs to be shown in order for
us to locate correct hydrants.
Must indicate which hydrant to give static/residual results and which hydrant to give flow results.
FAILURE TO PROVIDE ACCURATE COMPLETE INFORMATION AND PAYMENT WILL DELAY YOUR REQUEST.
