Under state law, a vehicle is considered to be a Qualified Highway Vehicle and subject to a permit fee if it is non-recreational and
has one of the following specifications. Check the one specification that best describes your vehicle

2 axles and gross vehicle weight or registered gross vehicle weight greater than 26,000 pounds, or
3 or more axles regardless of weight, or
a gross vehicle weight/registered gross vehicle weight greater than 26,000 pounds when used in combination.
Not a Qualified Highway Vehicle

Purpose: Use this form to apply for a permit to operate a vehicle with out-of-state license plates in Virginia while under contract to
harvest and/or haul crops to a storage house, packing plant, market or transportation terminal:
- as an incidental part of harvesting operations
- not to exceed a distance of 20 miles and
- when the use is a seasonal operation
Instructions: Applicant and Farm owner/lessee complete and sign their respective sections. Applicant submits completed
application and presents vehicle registration to any DMV customer service center to receive a Harvest Permit (RDT 387).
Harvest permits are valid for 90 days from the date of issuance.

a distance not more than 20 miles. I understand that this vehicle must not be used to transport workers.
I further certify and affirm that all information presented in this form is true and correct, that any documents I have presented to DMV are genuine, and
that the information included in all supporting documentation is true and accurate. I make this certification and affirmation under penalty of perjury and I
understand that knowingly making a false statement or representation on this form is a criminal violation.
