All financial aid students are awarded at a full-time enrollment level (12+ credits). Financial aid will not pay your tuition if you are
enrolled for fewer than 12 credits, unless you submit this form to the Financial Aid Office as notification. It is the responsibility of
the student to notify the Financial Aid Office if you will be less than 12 credits so that your tuition can be paid by the tuition
deadline, or thereafter; and you can avoid being dropped from your classes for non-payment.
You may also use this form to request a cancellation or reduction in grants, loans or work-study for a particular quarter of
enrollment. Withdrawing from classes must be done at the Registration Office. Cancelling your financial aid award does not cancel
your registration.
