1) The instructor made this offer available to all the students in the course; AND
2) No students were inconvenienced by the new requirements extending the coursework time period beyond
that which the university originally published.

This form is to be used for courses offered by the School of Accounting, Ageno School of Business, Braden School of
Taxation, and Undergraduate Programs. Law School grade changes must be submitted on a different form available
via GGU4YOU or from the Registrar’s Office.
To change a student’s grade, fill out this form, scan it, and submit it directly to the Registrar’s Office at
records@ggu.edu or fax to (415) 442-7223. Please keep a copy of the completed form for your own records. Both
you and the student will be notified by e-mail when the grade has been changed.
