Standards of Conduct for Educator Licensure Candidates
Statement of Assurance of Good Moral Character
A person shall be deemed to be of “good moral character” provided that said individual has not pled guilty to, or been convicted of
any felony and/or misdemeanor delineated by the Ohio Revised Code or other comparable municipal, State or Federal offense. Some
offenses are permanently disqualifying for educator licensing or employment, ref. O.R.C. 3319.31. Other offenses may be deemed
subject to rehabilitation, but are subject to strict guidelines and determination by higher authorities. See conditions specified in rule
3301-20-01 of the Ohio Administrative Code. Permanently disqualifying offenses generally include crimes of violence, sex offenses,
and other felonies. An explanation and listing of explicit criminal offenses barring or potentially prohibiting licensure can be found at

Student signature of this form shall be considered evidence that the student has read these materials and thereby acknowledges their
content. Moreover, in each academic year in which a student shall be placed in a field experience, the student shall consent to a
criminal background check.

In certain instances, the SEHS may determine that students are not eligible to be placed in a school setting as representative of the
University of Dayton and Marianist principles. To this end, the School of Education & Health Sciences further requires all students
pursuing programs at both the undergraduate and graduate levels leading to State of Ohio educator licensure to answer each of the
following questions:
1. Have you ever been convicted of, found guilty of, pled guilty to, or pled no contest to any misdemeanor other than a traffic offense?

2. Have you ever been convicted of, found guilty of, pled guilty to, or pled no contest to any felony?
□Yes □No
3. Have you ever had a criminal conviction sealed or expunged? □Yes □No
4. Have you ever had ANY professional certificate (license), or permit, or an application for same, revoked, suspended, limited, or
denied? □Yes □No
5. Have you ever surrendered ANY certificate, license, or permit? □Yes □No

This document, with original applicant signature, must be returned to the appropriate departmental office; e.g., with the EDT 110/507 Application for
Admission to the Teacher Licensure Program in the Department of Teacher Education. A copy is also required when undergraduate and graduate
students submit Clinical Application/Student Teaching Application forms.
