Coronavirus.Utah.gov/Vaccine
The vaccine is FREE for everyone. If you have insurance, vaccine providers can bill your insurance a fee
to give the shot, but you shouldn’t be charged anything or be billed later on.
Everyone can get vaccinated. You DON’T have to have health insurance, or be a US citizen or
resident. You don’t need a social security number to get vaccinated, but providers may ask for one so
they can get reimbursed for vaccines.
Let the vaccine provider know if you will need accommodations (such as language services, a wheelchair,
or someone to help you get around the facility).
You have to bring something with you that has your name on it (like a photo ID, utility bill, or a signed
letter from someone saying you are who you say you are).
