Coronavirus.Utah.gov/Vaccine
• The vaccine is FREE for everyone. If you have insurance, vaccine providers can bill your insurance a fee
to give the shot, but you shouldn’t be charged anything or be billed later on.
• Everyone can get vaccinated. You DON’T have to have health insurance, or be a US citizen or
resident. You don’t need a social security number to get vaccinated, but providers may ask for one so
they can get reimbursed for vaccines.
• Let the vaccine provider know if you will need accommodations (such as language services, a wheelchair,
or someone to help you get around the facility).
• You have to bring something with you that has your name on it (like a photo ID, utility bill, or a signed
letter from someone saying you are who you say you are).
