If an alleged incapacitated person (AIP) or incapacitated person (IP) needs a certified professional
guardian (CPG), but you are unable to locate one, please submit this form. AOC staff will email a brief
case description to the CPG listserv. CPGs interested in accepting the appointment will be instructed to
contact you directly. As a best practice, the Guardian Program would prefer to receive this form from the
guardian ad litem (GAL) appointed to investigate the need for a guardianship.
